Ambient AI Scribes: The Five Questions to Ask Before Signing

The pilot will look excellent. These are the questions the demo does not answer.

The demo is not the deployment

Ambient documentation tools genuinely reduce after-hours charting for many clinicians, and the demonstrations are persuasive because they are run on clean audio, cooperative patients, and a single specialty. Your Tuesday afternoon is none of those things.

The point of the questions below is not skepticism for its own sake. It is to make the contract match the clinic rather than the demo.

The five questions

First: who is accountable for the note? The signing clinician always is, so ask what the review workflow looks like when the draft is wrong, and how often that happens in your specialty rather than in aggregate.

Second: where does the audio go, how long is it retained, and is it used for model training? Get the answer in the agreement, not the sales deck. Third: what happens to accuracy with accented speech, interpreters, and multi-speaker rooms — ask for specialty-specific and demographic-specific error data.

Fourth: what is the exit? If you leave in year two, do your historical notes and templates come with you in a usable format? Fifth: who pays when the vendor's error contributes to a billing or safety event?

Pilot design that tells the truth

Run the pilot across at least three clinicians with different documentation styles, include your busiest session rather than your calmest, and measure edit time per note as well as satisfaction. Satisfaction alone rises simply because something changed.

Set the decision criteria before the pilot begins. Written in advance, they survive the enthusiasm of week two.
